I have unzipped version 1.2.1 and I don't see .htaccess file anywhere. What am I doing wrong?

Within question2answer folder, I see:
changelog.html
index.php
licensehtml
qa-config-example.html
readme.html
5 other folders

it might be hidden on your system. enable show hidden files feature in your explore.

tools->folder options->view->show hidden files and folder.

or if you are using linux/unix try below command
# ls -la
