After I click the button [Create Database etc.] here:

I get:
Forbidden
You don't have permission to access /index.php on this server.
I chmodded index.php to 777 … but I still get Forbidden. Please help I’m really stuck here!
BTW there should be a .htaccess file but it didn’t come with my Q2A zip file.
