How can I create a placeholder for textboxes?

I want to add text using the placeholder attribute inside textboxes like "type your question here" for question title textbox, "enter your password here" in the Password box and so on...

So, which files do I need to modify to add placeholders to the textboxes

Question box in Ask question page is not mobile friendly. Any solution ?
