To place it under the top menu, the easiest way would be the Widget Anywhere plugin: https://github.com/svivian/q2a-widget-anywhere

For adding between questions you'll need to use a custom (advanced) theme. Override the q_list function and output the code there. See http://question2answer.org/themes.php
