antibot captcha not displaying image

I am using the anti-bot captcha and the image does not display.

 

I sometimes get the following error in my logs:

 

PHP Notice:  Undefined index: IMAGE_CODE in /home/jobin/public_html/qa-plugin/capt/AntiBotCaptcha.php on line 72
 

any ideas?
