How to edit URL of post (question pages)?

I want to change the URLs of all the question pages. On my website every question page has a unique TAG.

For example my current question page URL = http://www.example.com/101/who-is-author-of-harry-potter     Unique TAG for this page = Harry Potter

 

Now I want to make my URL as => http://www.example.com/101/harry-potter

I want to accomplish this for every question page URLs.

(Consider me as a novice in your explanation)
