Need to used existing design.php as my template

I use a basic templating system on my site that uses a design.php file. Then in each file (eg index.php) I set the $maincontent and include design.php

 

is there any way of keeping this system and integrating Q2A?

It would depend on the details of design.php - I recommend you read up on using advanced themes with Q2A.
