Effective way to deal with Spam

Question

While i was doing some research on fighting spams i came across a google solution, it seems VERY effective :

From google official blog : 

If you're a blogger (or a blog reader), you're painfully familiar with people who try to raise their own websites' search engine rankings by submitting linked blog comments like "Visit my discount pharmaceuticals site." This is called comment spam, we don't like it either, and we've been testing a new tag that blocks it. From now on, when Google sees the attribute (rel="nofollow") on hyperlinks, those links won't get any credit when we rank websites in our search results. This isn't a negative vote for the site where the comment was posted; it's just a way to make sure that spammers get no benefit from abusing public areas like blog comments, trackbacks, and referrer lists.

the solution is to add automatically a "rel="nofollow" tag to the links that people may put. This should applies to CKE and WMD editors.

As far as I know this is allready in place here on q2a. Just see the page source and the outgoing links.

Besides this solution is now around several years, but didn´t help much against spam. In the beginning google prevented that unwanted pages got linkjuice (pagerank) through the these links, but spammers do not really need that, they need traffic. The more traffic the more income.

q2a has very good protection, a few details will still be optimized, but spam will always be a problem on interactive pages.

if you may please what are some of the hanging details ?

"a few details" may be said to much.
What I actually see is, that users shouldnt be able to register from blocked ips.
They still can do so, but they cannot do anything on the page, as long one doesnt confirm them.

The other thing, I would like, that spammers if they use forbidden words, get blocked and not that the bad words get changed into stars *****. This does not really help me.

Everything else seems to be very well actually...but...Spammers never sleep !
