guess this is for a plugin or advanced theme...

check out this example: http://qz.com/111748/you-can-now-leave-annotations-in-the-margins-of-quartz/ (comments per paragraph)

great stuff in my recent view :)

The how-to in general: http://stackoverflow.com/questions/19477494/per-paragraph-commenting-system

Interesting idea, but I'm not sure it would be appropriate for Q2A. Most questions and answers are fairly short (only a few paragraphs at most) as opposed to long blog articles as they are intended for. Comments at the bottom won't be far from what they refer to.

Plus, given posts are often edited, where would comments on one section go if the section is removed or split?

Here is an example where q2a is used for mostly longer posts: http://www.schreibgenuss.de/

I will try to implement inline-disqus there: http://tsi.github.io/inlineDisqussions/

Problem with the inlinedisqus-script is - as you said - after somebody changed the paragraphs, the comments will not be at the right position anymore.
